New patients, or previous patients visiting us for a new medical problem, will be asked to complete registration and medical history information. If you have an upcoming appointment in our office, you can download the registration forms; bring the completed forms with you the day of your appointment - do not mail them. You must bring your health insurance cards. Please arrive 30 minutes prior to the time of your scheduled appointment. IT IS ALSO HELPFUL IF YOU BRING ANY PREVIOUS MEDICAL RECORDS AND/OR X-RAYS REGARDING YOUR CONDITION. If you come to the office without these films, it may be necessary to reschedule your appointment. If you have trouble obtaining these films, please contact our office as soon as possible

APPOINTMENTS

Patients are seen in the office by appointment. To schedule an appointment at our Abraham Flexner Way office, call (502) 584-3376, during regular office hours. In addition, Dr. John Gormley also sees patients at our Kresge Way office. To schedule an appointment at this location, call (502) 584-3376, during regular office hours. To schedule an appointment with Dr. John Shaw at our Southern Indiana office, call (812) 941-6156 during regular office hours.

TELEPHONE CALLS & PRESCRIPTIONS

If you have any questions regarding your care, or are in need of medication, please call during regular office hours when we have access to your medical record. If the office is closed, the answering service will forward your message to the physician on call. Medication refills will only be provided during the hours of 8 a.m. - 3 p.m., Monday through Friday. Please have your pharmacy call during normal business hours only. After hours and on weekends, the physician on call will NOT be able to prescribe medication or grant medication refills.

INSURANCE INFORMATION & PAYMENT POLICIES

You must provide us with copies of all insurance information. If you are being seen for a workers’ compensation injury or illness, or a motor vehicle accident, you must provide all contact and billing information, otherwise you will be responsible for your bill.

The fee for treatment will vary depending upon the complexity of your problem. Other services such as diagnostic testing, casts, splints, injections, etc., involve additional costs. If you have not met your insurance deductible or have no insurance, we request payment at the time of service. Co-payments, if required by your insurance, are due at the time of service. If you cannot pay at the time of service, please discuss this with our staff and payment arrangements can be made. We do accept VISA and Mastercard. We do accept Medicare assignment and participate in most insurance plans. If your insurance plan will only allow you to see physicians who are members of their network, please verify that we do participate with your particular plan prior to receiving treatment in our office. If your insurance plan requires you to obtain a referral from your primary care physician, it is your responsibility to obtain this referral prior to receiving treatment in our office. If you are scheduled to undergo surgery, please check your insurance policy to determine if a prior authorization or second opinion is required. It is your responsibility to know your plan requirements, and to pay the deductible, coinsurance, and any other non-covered service not paid by your insurance. We will file your insurance claim, but you are responsible for your bill.

Please allow up to seven days for completion of disability insurance forms. There is a $10.00 fee for each form, and payment is required prior to completion. If the form requires completion by a physician, there may be an additional charge.
